One of the certificates that an applicant who lacks an address proof document may use is Annexure A. A member of the Legislative Assembly, a member of Parliament, a municipal councillor, or a Gazetted Officer should fill out this form.
Who can issue Annexure A for the PAN card?

What is Annexure B in PAN card?

Procedure to Apply for a Verification Certificate
On the official website of the IT department, the format for the verification certificate is listed as part of the terms of rule 114(4) of the Income Tax Rules, 1962 of PAN (individual/HUF).

Information Required from the Applicant
