One of the certificates that an applicant who lacks an address proof document may use is Annexure A. A member of the Legislative Assembly, a member of Parliament, a municipal councillor, or a Gazetted Officer should fill out this form.

Who can issue Annexure A for the PAN card?

This form must be completed by the municipal councillor, MLA, or gazetted officer. The applicant must attach a passport-size photo to the right-hand corner of the application, then read and keep in mind the following information to understand how to complete Annexure A for PAN Cards.

  • Name of the Father
  • D.O.B.
  • Address of the Permanent Residence
  • Address of the Current Office
  • Change of Name (in case one has changed his/her name)

It is necessary to fill out a declaration that identifies the issuing authority as the applicant’s certifier. The applicant must then sign and have an official rubber stamp placed on their photo to authenticate it. The application form must have half of the signature and stamp, and the photo must have half of both. The issuing officer must additionally include the following details on the form:

  • Issuer’s Signature
  • Name

What is Annexure B in PAN card?

PAN cards use Annexure B, which is nearly identical to Annexure A. If the petitioner does not have proof of address in this case, they may just provide Annexure B. On the right-hand side of Annexure B, the applicant’s passport-sized photo and residential address must also be attached.

The employer must then complete the following information to confirm the validity of the applicant’s address.

  • Declaration statement with the necessary information
  • Address of the Office
  • Company’s registration number
  • Organisation’s PAN details
  • Issuing authority’s complete name
  • Designation
  • Individual’s PAN card details
  • Location and address of the office
  • Contact Number
  • Signature and the seal of the issuing authority

Once completed and validated, the verification certificates stated above will serve as confirmation of both identity and address.
